Deep endometriosis is a chronic and inflammatory condition characterized by the presence of functional endometrial tissue outside the uterine cavity and myometrium. This endometrial ectopia leads to significant clinical repercussions, particularly regarding impaired reproductive function, often associated with infertility. In this context, videolaparoscopy represents a key surgical tool for reducing morbidity and achieving better outcomes in women with endometriosis who wish to conceive. The present study aims to conduct a literature review in order to gather and interpret relevant data on the effectiveness of videolaparoscopy in reducing morbidity and improving fertility in patients diagnosed with deep endometriosis, from 2019 to 2024. This is a narrative literature review based on scientific articles and book chapters published in the last five years. The databases used were PubMed and Google Scholar. The descriptors used, in both Portuguese and English, included: "deep endometriosis", "videolaparoscopy", "morbidity reduction", and "fertility". The search was conducted systematically using the aforementioned terms. Initially, 1,298 articles were identified on PubMed and 1,740 on Google Scholar. After reading titles and abstracts, 34 articles were selected based on established inclusion criteria. Of these, 15 were included in the final analysis, allowing for a comparison of existing consensus and disagreements in the literature. Endometriosis is a highly complex gynecological condition with a significant impact on women's quality of life, especially concerning reproductive health. Therefore, the adoption of early and multidisciplinary therapeutic strategies becomes essential for effective management. In the context of surgical treatment, videolaparoscopy has been established as an effective approach, providing improved reproductive outcomes both in spontaneous conception and in vitro fertilization (IVF) treatments.
