Increased life expectancy and advances in diagnostic methods have contributed to the rise in the prevalence of chronic diseases, such as cancer, a rising cause of death in Brazil. This study analyzes the correlation between Years of Potential Life Lost (YPLL) due to neoplasms and cancer care coverage in the state of Sergipe, from 2020 to 2023. This is an observational, descriptive, and quantitative study based on secondary data from DATASUS. Indicators of incidence, mortality, and cancer care were evaluated, with YPLL calculated by age, sex, and municipality. Ethics approval was not required because the information is in the public domain. The results showed 56,855.5 PYLL due to cancer in Sergipe, corresponding to an average rate of 6.73 potential years of life lost per 1,000 inhabitants, between 2020 and 2023. The temporal analysis showed that 2023 presented the highest values, totaling approximately 15,653.5 PYLL, followed by 2020 (14,504 PYLL), 2022 (13,552.5 PYLL), and 2021 (13,145.5 PYLL). Regarding age groups, the 50-59 and 40-49 age groups contributed the most to the total potential years of life lost, while children and adolescents presented the lowest values. It was also observed that the Sergipe territory experienced an increase in PYLL in parallel with the concentration of the main cancer treatment centers in the capital, Aracaju. Thus, it is concluded that, even with the expansion of healthcare coverage, early mortality indicators remain high, reinforcing the need to decentralize oncology services and to use PYLL as a strategic tool for planning public policies that promote greater equity in care.
