Relationship between symptoms, nutritional status and nutritional monitoring in the clinical outcome of patients with gastrointestinal cancer undergoing chemotherapy

Abstract

Objective: To evaluate the relationship between symptoms, nutritional status and nutritional monitoring in the clinical outcome of gastrointestinal cancer patients undergoing chemotherapy treatment. Methodology: This is a descriptive and analytical cross-sectional study that included patients with gastrointestinal cancer under chemotherapy treatment, assisted at the Outpatient Clinic of the Instituto Hospital de Base do Distrito Federal (IHBDF) between August and September 2022. Data were collected using a semi-structured questionnaire prepared by the researcher, the Subjective Global Assessment Produced by the Patient Himself (ASG-PPP) and the IHBDF electronic medical record system. Analysis of variance with Poisson distribution was used to evaluate the mean total symptom score among nutritional statuses (AS-A, B and C), logistic regressions to analyze the relationship between categories and nutritional statuses and symptoms, and Chi-square test for follow-up and nutritional status groups. A posteriori analyses occurred to identify which groups differed from each other (P<0.05). All conducted using R version 4.1.3 software (R Core Team 2021). Results: Ninety-two patients were included, with gastrointestinal cancer under chemotherapy treatment, mean age of 60 years, mostly male (60.9%), married (44.6%), low education (50%) and family income (63%), diagnosed with intestinal and rectal cancer (51%), metastatic (54.34%), advanced (stage IV) (53.3%) and nutritional status: malnourished (70.6%). Three symptoms were related to the nutritional status of malnutrition (P<0.05). Regarding nutritional follow-up, significant differences (P <0.0001) were found among the categories of nutritional status, and the frequency of follow-up was higher (70%) in the ASG-C group. Conclusion: The results of this study demonstrated the existence of a direct relationship between the presence of symptoms, nutritional status of malnutrition and absence of nutritional monitoring, with unfavorable clinical outcomes in gastrointestinal cancer patients undergoing chemotherapy treatment.
