Introduction: Patient safety is a relatively recent issue in the Brazilian scenario and still in constant development. Among the international safety goals, the safe identification of pediatric patients occupies a prominent role, due to the vulnerability of children and the failures in communication and standardization processes. Objective: To analyze, based on the scientific literature, the challenges and strategies aimed at the safe identification of pediatric patients. Method: Integrative review, with the following guiding question: "How to ensure the safe identification of pediatric and newborn patients, considering their particularities and vulnerabilities, in order to reduce risks and promote safer care?". Results: 188 articles were identified, and after applying exclusion criteria, 10 articles were used for the research. The articles were divided into two thematic classes: challenges and strategies in the identification of newborns in neonatal intensive care units and standardization, innovation, and regulation in the safety of pediatric identification. Conclusion: Correct patient identification is an indispensable step to guarantee the safety and quality of care provided, especially in pediatrics, where vulnerability is greater. Therefore, it is essential that professionals adopt standardized practices, maintain clear communication, and value the participation of family members in the care process. This makes it possible to reduce risks, strengthen trust in the team, and ensure care based on ethics, responsibility, and respect for the life and integrity of the child.
